I can't say what you are doing wrong here as I do not have that much experience with PS.
You could achieve something simular by making a form sheet in MS Word. Send me a list of the data you want listed and I'll make a set up for you. So I have an idea of the size you are looking for, do you wish full size pictures or 2 or 4 pictures w/ data on a sheet of paper?
