Question from my wife that I don't have any clue as to the answer.

Question is - Which island in the Caribbean or country in Central America has the largest US Expat population?

Well, this paper: http://www.realtor.org/Research.nsf/files/07AmericansAbroad.pdf/$FILE/07AmericansAbroad.pdf
suggests Mexico followed by the Dominican Republic, Costa Rica and Panama.
 
And of course neither the USVI nor PR are listed as they are both nomially part of the US, so any Americans from the mainland living there would not be considered expats any more than somebody from Minnesota who has moved to Florida would be considered an expat.

Further, in terms of the stats for Mexico and the DR, for example, the size of the country matters, so while Mexico may have more expat Americans in raw numbers, it isn't hard to find places where there are none at all in a community; this would be less the case in the DR.
