Absolutely Never.

A ScubaPro cam band, once properly adjusted, never slips.
I see no reason to even try anything else.

If you have a non-ScubaPro BC, you might be able to refit it with a SP band. The band can be bought at a ScubaPro LDS. The refitting is easiest on generic hard backpack BC's that just have the band thread through a slot and secured with a couple of clips...if its this configuration, its an easy 15 minute DIY.

Nope however I use a Scubapro tank band and I have yet to see a SPro tank band (that has been properly mounted on the tank) slip. I've seen it happen to plenty of the plastic cam bands though.
 
I did for the first season that I had my Transpac, but now I don't bother. I switch between 72s, 80s, 95s and 100s and haven't had a problem...yet.

As many others have pointed out double cams rule. :bounce:
 
I don't, I was told scuba pro bands don't need to and have not had a problem. In fact, the buckle clamps so tight you need to watch your fingers.
 
I always set up my rig at home before heading out and I usually spray the cam bands with water before - much easier to transport fully setup. If I don't wet the bands then I notice that they are they are much easier to open after a dive then it was to tighten them - haven't had a tank slip though (Pioneer setup).
 
double straps with steel buckles and a rubber sleeve always seems to help and I haven't had a slippage yet.
Buddy with single strap, plastic buckle and no rubber sleeve, has had slippage and needs to prewet his.
