@tridacna, is she going to New York Eye and Ear vestibular rehab? They are awesome. I also can give you some good neurootologist referrals if she is considering surgery.
What surgical options are there for PPPD? My wife so far has tried VRT, multiple prescription options, even tai chi has helped her with balance. But we haven't been presented with surgical options yet.

I don't do these procedures myself, but from what I understand the two common procedures are cutting of the singular nerve (part of the vestibular nerve), or blocking one of the semicircular canals.
