I just came back from a liveaboard to similians and richelieu rock. Beautiful diving with great coral and sea life. Definitely worth doing. I want to go again! There are currents and surges at some sites, but I found them manageable and I generally struggle with current. Go between the full/new moons for less current.
 
I also recently returned from a liveaboard to the Similans, which included Koh Bon, Koh Tachai and Richelieu Rock. Those last three areas were definitely the best of the trip, but the other islands are doing well these days. Much of the marine life has recovered from the Tsunami and bleaching of a few years ago. The Southern islands in the chain simply exhibit one of the stages of coral re-growth that is the normal cycle of recovery. There is much less evidence these days of the old practice of dynamite fishing, though that also took a toll.

Many of the boats will offer more advanced courses that you can take while on the cruise. Nitrox is very worthwhile, as is the Deep specialty.
