That sounds like a good plan, except that you really can't do it in many cases. There are dive destinations all over the world in which only a few (or sometimes even one) agencies are represented, and if the one you prefer at home is not one of them, you have little choice.

No it is a good plan. He didn't say stick with one for all certs--agree that may not be possible. Sticking with the same agency for your open water cert is a very good idea and may have eliminated this problem. It is also my belief that it is the responsibility of the referring instructor to make contact with the shop or instructor that will complete the referral and make sure everything is arranged ahead of time to prevent problems.
 
From the 2018 manual, it looks like it would be the instructors responsibility that they have the Padi version of diving knowledge verified by some written evaluation method. Then, there is the issue of the confined water session skills, and lastly the open water dives.

That is coming pretty close to an entire course minus some background theory. It also sounds like the previous “experienced diver” cert that Padi instructors used to be able to issue.

What happens if they fail the exam or quick review or are not familiar with the Padi version of doing things? You could be back to doing the entire course over again.

If the aggressor instructor signed off on the open water dives in their logbooks, his number and hence his identity should be able to be found. PADI should have his email address on record.
