Did you decide on Cebu or Manila as your gateway? Definitely you can still fit together all your wishes in that time, and do a bit of hiking/cultural stuff on the day before you have to fly within the Philippines. Also, are your tastes in accommodation midrange or budget?
 
Flying in through manila would be a great option if you arrive in the earlier part of the day, like that you can take a domestic flight the same day to either Dumaguete or Tagbilaran(Bohol). Flying in to Cebu is also an option, but will take around 4-5hours to Dumaguete or around 4 hours to Malapascua. Moalboal would be the nearest destination at around 2 hours by car. 3 weeks is a long time so for your you wanna go island hopping. 1. Dauin/Dumuagete- Apo Island coral gardens (30 minutes boat ride), whalesharks in oslob (90minute boat ride), and great macro diving around the coastal area. Be aware of the different diving/sanctuary/government fees that might not be including in dive package in some dive operators. Excursions to markets and lakes are also possible there. check out Pura Vida Beach Resort2. Cabilao/Bohol - Beautiful wall dives, small quiet island, perfect for relaxing and diving. From there you can also do excursions to the chocolate hills, visit the tarsiers, etc. Pura Vida Cabilao (formerly Cabilao Beach Club)3. Malapascua- Thresher sharks, day trip to calanggaman island (google image search it, its awesome) gato island, some white and black tips around as well. Resort: Ocean Vida Beach Resort I agree with Centrals that combining Dauin and Anilao would be too time consuming while you have the option to do other great combinations nearby. My recommendation would be to check out Sea Explorers who have dive centers and resorts in all the abovementioned destinations. They also do dive boat transfers between Cabilao and Dauin, so you can make 2 dives on the way including whalesharks in oslob and balicasag Cheers
