On the document, it says 'not a scuba related incident' - I had gotten an email from Wayne regarding her passing, but no information as to the cause.
 
That wasn't very helpful.

Chantal's name and entire incident report has been removed in the last 24 hours from the OUC 2011 Incident Report that I linked to above. It was there 25 hours ago when I posted it. It does not seem very professional to simply remove the incident after it was investigated as a scuba accident and made public. Chantal was found on a Friday after being missing since Wednesday. She was found drowned in 7 feet of water and partially clad in scuba gear. If Chantal's death has been deemed to be a suicide as some people think or due to some other cause, then you could perhaps write that in a follow-up to the report.

That statement "not a scuba related incident" seems to have been before it was removed entirely from the document.

Wayne and Mark sent emails. Wayne also posted his email a couple of weeks ago on Ontario Diving and someone else posted it on the passings forum on Scubaboard. What Wayne said in the email and post regarding Chantal's death was:

Wow - amazing. Yeah, I saw it when you first posted it. Now it's just gone. :confused:
 
Another good reason to post the article in addition to the link. Don't know if it was possible in this situation though since I don't know the format of the original report.

Same PDF format. We see news stories vanish on the net but who would have thought that they'd pull this?

Date of Incident: 2011-06-10
Summary:
Ms. Chantal Theriault of Rockport, ON was found on Friday after having been missing for a week. Ms. Theriault was the life partner of Wayne Green of Thousand Islands Pleasure Diving. According to Mr. Green, Chantal was found with scuba gear on, in 7 feet of water, at Point Traverse.
No information is available at this time as to the cause of death. A coroner’s investigation is expected. Police are treating this as an accident at this time.
This incident will be updated if more information becomes available.
This incident is not related to a near miss which occurred on the weekend. See notes below under near misses.
OUC Recommendations:
 Do not dive solo.
 Inform someone of your dive plans and expected time of return.
