700C x 18 or smaller works well. Bike shops give them away...
 
Tubular webbing. You can find it at just about any outdoor store. I then use plastic tubing to cover the screw
 
I was having a mental block earlier. I just went to a local store, but REI should have reasonable prices.

I use "tubular webbing". I get it at REI. It's the same stuff sold in the Stage Kits from Halcyon.
