I had my most of my cervical spine removed and my head bolted back on with titanium rods. Couldn't dive for 8 months and then only with great difficulty for a while. Very intensive physical surgery. Work with your medical team to ensure that you are really ready to dive w/o limitations and then don't go out of your way to disclose it to dive operators who don't know you.

It might make sense to have your physician complete a PADI medical clearance and then carry that in dive log.
