Hi,
I am looking at the Apek's ATX50 but understand I need to use a ½ MALE TO 3/8 FEMALE adapter to use 3/8" hose. DO I need 4 of these fittings for this 1st stage or just 1 adapter?

On the LP ports, I mean...
 
Apeks has a large range of first stages that are all very similar. The differences are:

number of LP ports (1 or 4)
environmentally sealed or not (DS or US)
turret or not (T)

eg US1, DST, DS4

The ATX50 will almost certainly come with a DST first stage (4 LP ports, one with 1/2", and a turret). Some shops will sell it with DS4 (No turret and four 3/8" LP ports), you might have to ask nicely.

Personally I think the turret doesn't offer any benefit on a back gas reg.

All of these first stages are available with DIN or A clamp (just specify when ordering). The DIN/A clamp fitting just screws into the body so it is easy to change at a later date, anyway.
 
You only need a converter if you intend to swap out the Apeks 1st to 2nd stage low pressure hose as it comes from the manufacturer. The 1/2" port is only on the 1st stage, and then only on the port that the 2nd stage is connected to.

If you don't carry around a spare Apeks regulator hose, then swapping it out for a "standard" low pressure hose is advisable, as it will allow you to "borrow" a spare hose from someone else if needed.

I have two Apeks regulators I dive with set up as redundant 1st stages on either a manifold or h valve. One set with the 2nd stage connected to a standard port on the 1st stage and the 1/2" port plugged. I carry around the hose I took off of that reg as a "spare" for my other Apeks reg set, in addition to a normal medium length low pressure hose and an 1/2" to 3/8" port adaptor, and few spare 1/2" and 3/8" plugs. This insures that if there is a need, I have two possible spares to use with either regulator set, or have one spare to loan out.
 
I'll put the 1/2" to 3/8" adapter on that I already bought...my 6ft LP tech hose is 3/8" and I want to use that LP hose.
 
Last time I tried to buy an Apeks reg from LeisurePro (two actually), they did not carry them in DIN, and were not able to get them in - so double check before ordering from them if that is what you want.

Isn't the DS4 a DIN 1st stage?

It's available in both DIN and "Yoke"/"A-clamp" outside the U.S.
