I don't know if this is relevant to your problem, but here's my saga of how to screw up the Olympus XD card used with an Olympus Stylus 400 camera.
I put the XD card in a built in reader in my computer and somehow ended up writing to it. The computer changed the card format (From Fat12 to Fat16, or something like that.).
I found some program to go into the raw data and extract photos and retrieved just about everything. But my camera wouldn't format the card, and reformatting on the PC just resulted in "Card Error" on the camera.
It turns out that the Olympus brand XD card has some extra info that IDs it as Olympus. Some Olympus cameras will only do panoramic mode with Oympus brand XD cards. To prevent you from doing it with other cards, they will not reformat.
In searching the Internet camera forums I found the simple solution ----- Reformat the card on a FUJI camera. It uses the same format (FAT12 ??) as Olympus cameras, and isn't finicky about which sort of card you want to reformat.
I wandered down to Long's Drugs in Kihei, asked to look at one of the Fuji cameras, popped in my XD card and reformatted. Worked fine after that.
Nowdays, I always use the camera USB port to download and erase photos.
If you have another card around, pop it into your computer card reader, click My Computer, then do right click and "properties" on the card. Look at "file system" type. Then do the same with the card that is giving you trouble. If they aren't the same file system type, then that's a problem.
