Got me a Lavacore hooded vest before quarry opened in April. I dived it 2 days, no problem. Third time was not the charm! I got the dreaded "bubble head!" Oh the noes! :O I had such a bubble head that my buoyancy and trim were really messed up. Had to add a couple of pounds to get down!

The only thing that was changed is that I didn't wear my 5mm Waterproof hood on top of vest hood since the water was a bit warmer. I was squeezing air out of my hood all through the dive. 2nd dive put my Waterproof hood back on top. Still had the bubble head problem. But I was warmer.

First Lake Michigan dives of the season Sunday, so I need to poke some holes in my Lavacore hood.

No clue why I had 2 days of dives with no bubble head and why I did on the third day. Weird.
 
Hot nail will make nice holes in the top back area and fix this
 
Hot nail or a soldering iron works a treat.

I found a very fine soldering iron tip to give me the best results. A small series of holes covering a wider area seemed to work better than a larger hole in my experience.
 
At one point Henderson put a relief valve in the top pf their hoods--which did not work. But it did give an aiming point for the center hole that I burned in the hood. Then I added a few more fore and aft.
 
My Waterproof hoods have a nice valve. But not the Lavacore, obviously!

In California divers put on the hood mark to ear locations with a felt tipped pen, remove the hood and heat a small nail to red hot and thrust the nail though the hole several times in an acceptable pattern.

If the bubble head persists you might want to purchase a duck bill valve from vintage double hose and have it installed in the top of your hood as a simple but effective method of combating that dreadful diving problem of bubble head .

I saw this happen to a diver on 1 dive. Had a vent on the top of the hood but it could not keep up. The 3 other dives he made that weekend were no issue. Only thing we could guess as the cause was maybe he had the hood slightly twisted on the problem dive.

Maybe your 5mm hood squeezes the vest hood tighter around the face where the air was getting in.
 
you are diving head looking down. get horizontal, head back (neck flexed) bubbles go up not into hood
