For Cozumel, try Island Dreams(claudia@divetrip.com)for packages that include air, hotel (Fiesta Americana) and dive shop (The Dive House) on-site. Cozumel has great diving and is very reasonable.
I also think Hawaii is great. Both Maui and the Big Island have excellent diving. It is different than the Caribbean because there are really no soft corals. However, the sea life is wonderful! On the Big Island, two dive shops that are highly recommended are Sandwich Isle Divers and Jack's Dive Locker. On Maui, I think Ed Robinson's is the top operator, but Maui Dive Shop and Lahaina Divers are good shops also. Try Travel Hawaii (infinity@lava.net) for air/hotel/auto packages.
