What went wrong procedure wise?

Well, this was an exploration dive, and it's not clear from the article if the severed/lost line was laid by them or a previous dive/team. I would also add that the team separated, and regrouped "by chance", making it sould like perhaps they were improvising and not following a strict plan.

Far too little information to really determine root cause, but it sounds like a combination of lost line, buddy separation, silt out, and new, unfamiliar cave.

One thing seems clear, this was not your basic recreational cave dive.
