1960's US Divers Conshelf XI (yes between the X and XIV) with modern Aqualung 3000 PSI service yoke.
Regulator has TWO LP ports one HP port. No LP inflator hose (shown in photo) just LP hose from first to second stage.
Overhauled a year or so ago and only used at Vintage Portage Quarry event 2010 and maybe a couple of pool dives.
