What are the regulations on certifications? Do they need to be renewed and if so in what time frame?

If you get certified, you're certified for life. If you haven't been diving for a long time, it's a good idea to take a refresher, but that's a matter of good judgement, not law... at least in the US.

Generally, no...with at least one exception I can think of, but only at the technical level.
 
Renewal required every three years to dive in Quebec, and annually to divemaster or teach anywhere.

For most professional qualifications (instructor, asst. instructor or Divemaster) yearly renewals are required.
