Please Help Me ID My Regulator

Hey everyone. I am a former member who lost my account info/password several years ago. I was certified in 1997 and have been out of diving since roughly 2005. Fast forward to now.. I fell in love with a girl who dives and wouldn't you know it, I am getting back into diving. I purchased my current equipment in 2005 and cannot for the life of me remember the model name of my regulator. Can someone please help me ID it?

Digging a little deeper, I think it is a flathead, but does anyone have a clue as to what number?

I believe it is a "ZX" second stage.

From the service manual:

"The letter at the beginning of the serial number on the second stage indicates the model of that second stage. If the serial number begins with "H" (e.g. H001324) its is a second stage with venturi and breathing effort controls. For further information contact Zeagle."

You are correct. After digging in the attic I was able to finally locate the box.
