My son started his PADI training a couple of years ago. He passed the classroom and pool training but life got in the way and never did his OW cert dives. Can he take a refresher and proceed to his OW cert dives? Or, does he have to start over?

That would be up to the shop, but if it were me....or my son....I’d start over.
 
12 months is the maximum allowed delay. He will have to start over. He'll probably learn a lot more, and have more fun, the second time through than he did the first time!
 
A couple of years is a long time to go without practicing at all.

If it were me I wouldn't even allow him to pass go without taking the course all over again.
