The flash unit that came with my Olympus EM-5 failed in Cozumel. Luckily, I am able to borrow my wife's EPM-1 flash and keep shooting.

FWIW, carry a backup flash when you go on vacation.
 
Is this the FL-LM2? for the top shoe?
 
I believe that is what he is talking about. I looked at that flash once and thought about how how critical it was to dive photography and how hard it would be to replace on some island, so I picked up a second to take on dive trips. It takes up hardly any space and weighs almost nothing. They can be obtained new on ebay for about $25-30
 
Yes, the FL-LM2 that comes with the camera. With slave strobes, the camera's flash must fire to use the strobes. If I had known they could fail, I would have carried a spare.
 
Thanks to both of you, I am looking into getting a spare (slaved strobes). My wife doesn't do UW photgraphy so can't use hers.
