Minimum exposure protection.

Beyond wild Caribbean dive shop stories (100th dive naked, and other unmentionable shenanigans) is this more common than one would expect? Any gear considerations?

35 year veteran. Never seen nude diving. Don't want to.

One of my cave diving instructor trainer candidates was in the special forces of his country's military. A very famous celebrity couple was coming to swim in a natural underground pool with a cave system. His team was deployed as security to make sure the area was clear which included an underwater sweep. With their job done and the actress deciding to swim nude. They left the scene, but entered a nearby sinkhole to traverse to the pool in rebreathers via a tunnel to see her naked. They accomplished their mission, but were horrified to see that she was so skinny she looked like a concentration camp survivor. Rather than be voyeurs, they thought the team medic should start an IV and they should get her to a hospital or steal food from Ethiopian children to feed her.
 
There is a Russian freediver named Natalia Avseenko who dives naked under ice. Her videos are worth a look. :)

One of the LOBs we use has a regular group of nudists that books a week every year. To be honest I did not ask what they wore when diving...
 
She perished on a dive in 2015.
 
I wear exposure protection even in the warmest tropical waters mainly to avoid stingers but also not to frighten the marine critters
