Negril CSA diving form

I was reviewing the couples website and was a little confused by their PADI medical form requirements. It states that all divers must have a physician's sign off prior to diving. My experience to date (all other dive destinations) has been that you didn't have to have a physician's approval unless you had checked yes on the form. Does CSA require it regardless? Planning a trip there and need to know if I have to get in to see my physician prior to the trip. Anyone been there recently and can tell me their experience re: the approval process. I would be po'd to get down there and not be able to dive.
