I would like to know that too. We are talking about what is basically a fairly shallow open-water shore dive, so something out of the ordinary must have happened.

Coroners reports can take years in NSW. I asked Steve's former boss about it, not long after it happened, but he declined to talk about it, citing the coronial process.

Most deaths like this do not even get to the Coroner's Court. Over the past 34 years in NSW, I can think of only a half dozen that have.
