Ok, I've looked and I can't find where I read it but I thought it was here. There is supposed to be some new sort of physical form that I need my MD to fill out. Is it DOT or from the USCG?

Click on 'cg-719K' form. This is the one revised in 01-09--don't use any earlier form Also on the instructions link for using it. And download the other stuff if you don't already have it.
 
Found it. Only need to use that one when renewing a license so don't have to worry about it this year.

It is important to note that the Medical Form was revised January 1, 2010. The current form is now a 9 page document. The old medical form which is no longer accepted by the Coast Guard was a 4 page document. If for any reason you plan on using the old form be prepared to have your application denied...
