Generally, ear wax is a good thing and should be left alone unless it is truely impacted against the ear drum. Chronic cleaning can be a contributing factor in external ear infections.

If you do need to clean your ears you can pretty much do it yourself with one of those over-the-counter kits with the carbamide peroxide softener and bulb syringe or if you are really serious you can get the real-thing. It does not make a lot of sense to have a doctor or nurse do it who is not in the pain sensation feedback loop because it can be done too aggressively, especially if they are using a powered "waterpick" style device.

Also, earwax would have to have completely impacted and immobilized the eardrum to have any connection with middle ear equilization problems. This would be extremely rare.
