I was just getting my gear together for a dive Friday, I connected my BCD, inflator hose and First stage regulator. I began to pressure up and an o-ring on my 2nd stage hose connected to the first stage popped out at about 1800 psi. This is new equipment and hasn't been in the water but about 4 times. After further inspection the the fittings weren't tight on any of my hoses connected to the first stage! Remember your pre-dive safety checklists, pay attention to your equipment be safe and have fun in the water!
 
It was from a dive shop on the East Coast, right after my OW training back in July of last year. I have kept the equipment inside over the winter and was just bringing it back out and checking things out when I found the fittings in the 2nd stage were loose. This was a new BCD and regulator package from Oceanic and was put together there in their shop.
 
Properly torqued connections do not loosen without the use of tools. You should thank your dive shop for a very important lesson on inspecting your gear.
 
I carry a spray bottle of soapy water in my sav-a-dive box and spray all my air connections after setup. A oz of prevention is worth a lb of cure, or a short dive. :)
