How on earth do you not lose the lens cap that come with the lens? And is there a good way to add a lanyard so if (and when) I drop the lens, I actually have a chance to recover it?

I keep the lens cap in my lens bag that came with my lens (Sea & Sea, not Inon). If it didn't come with a bag, you can get them fairly cheaply at the local camera store or on the internet. You can find commercially available lanyards through UW suppliers that advertise on this forum, or make one. I think generally, they're looped around the narrowest part of the lens housing, just loose enough to let the lens spin. A lens dock isn't a bad idea either (mounted on your strobe arm, generally) though I just zip mine into the pocket of my BC. Two downsides to that are a)oops, I thought that pocket was zipped. b)oops, almost crushed my lens when I forgot it was in my pocket and dumped my rig onto the floor of the boat a bit carelessly.
Here's a link to the AD lens dock (holds two lenses between links of strobe arm)

I don't carry the lens cap for my Inon lens when I dive. I leave it onboard the boat. I dive with the Inon WAL in a small neoprene draw string sack that I attach to a D-ring on my BCD. The inside surface of the neoprene stuff sack is soft and does not scratch or marr the lens surface. When I want to switch from normal lens to wide angle I pull out the Inon and screw it onto the port of my Ikelite housing and away I go.

The lenes cap stays on the beach or boat and the lens is mounted at entry and exit. Attached are some photos of how I have a landyard rig onto the lens that runs to a shackel on the tray. You will also see a leash that runs from the shackel to a bolt snap that clips to my upper left D-ring.

For boat diving I clip the camera off on a line from a cleat near the entry point or have it handed down to me by the leash. Getting out it is the same way, either clip it off and get it latter or hand it up.

In the water I find that the landyard is long enough that I do a half rap around my hand and the lenes hangs just below the tray.

The leash is long enough so that I can drop the camera and it will hang just behind my left hip and under the tanks. This lets me use both hands when I need them.
