I believe IANTD was who published and distributed it. So you might send an inquiry to them if they know where you could get a copy.

It was printed by Amardillo Press, copyright 1997, but is not "published." The price tag on my copy says $37.95. The Hong Kong eBay page has it listed for $99.99 as a rare book.
