My understanding is that you can call PADI or go get an replacement card envelope from your LDS. I was told it is $25. Good Luck!

I just downloaded the application from NAUI and will be sending mine out tommorrow. I didn't lose mine, its just so thrashed from coming in and out of my wallet that its getting unreadable.

Advice: I scan all my C-Cards and keep them on a CD so that I can print a copy to send with the application if I ever lose my card.

Hi people
can you help me get a replacement PADI card?
how do i get one, i lost my other one.
Aatif

Phone PADI. You will need to fill in a form (which you can download from the internet), attach photos and money and send it to them. In order to verify your previous certification they will need your birth date and name.

If they have trouble finding you on record you can also let them search on the address where you lived when you were certified, the name and/or address of the dive centre or the instructor's name or number etc.

PADI's customer services are top notch and they'll help you any way they can.

Also, carful how you spell your name. My name was somehow put in funky( MC KINLEY, instead of MCKINLEY)

GOOD LUCK!

If you have the paper with your number on it, all you have to do is type that in on the PADI site and they have all the information right there for you. It was totally amazing when I upgraded my son from Jr. AOW to AOW. They knew everything about him. Hmm. Maybe this isn't a good thing? LOL

You can do it online with a credit card and a digital photo of yourself. That is how one of my buddies got his replaced.

The link is the same as Mark's above.
