Do you think there is an increased risk of pneumothorax or air trapping when you face vertically downwards? There is a reasonable amount of force of air forcing it's way to the surface and against the bases of your lungs?

I don't think so. As I understand fluids, pressure against walls (in this case lungs) is the same in each direction. Maybe it's too simplified to look at lungs as a bag but let's wait some real physiological explanation.
