I'm heading out on a cruise to Nassua, St. Thomas, San Juan, and Great Stirrup Cay. Can anyone recommend a reputable diving outfit, any favorite spots, or any must dive spots in these areas?

Admiralty Dive in St. Thomas is very well regarded. My wife & I had a great experience with them back in '06, and I've noticed they're well-spoken off on this board.

In Puerto Rico I dove one day with Sea Ventures and that was fine; they weren't located in San Juan, but our dive leader drove around & picked us up & took us to the dive shop in Fajardo, in eastern Puerto Rico, if memory serves. That, too, was back in '06, and I can't compare them to any other dive op.s on the island or tell you much else. Just that we used them that day & it was fine. Other posters may have better input to give.

Great Stirrup Cay is just a nice private beach, and there aren't really any dive ops I'm aware of that you could use. Additionally, AFAIK, the cruise line doesn't offer any diving there.

Bring your snorkel gear. It's still nice, and if you get away from the crowd, out by the breakwater, there's usually some interesting stuff to watch.

St. Thomas scuba diving was nice on St. Thomas and can pick you up very close to the ship.

Not sure about the other two.

On Nassau the cruise operator is Stuart Cove's. Also the biggest operator on the island. They pick up near the port and shuttle you 45mins. to their location. Stuart Cove's Cruise Ships Shark Cage Diving Bahamas Stuart Cove's Aqua Adventures - Cruise Ships

Bahama Divers is located a few minutes the other way - at the base of the bridge to Paradise Island. But depending on your arrival/customs clearance time, you may not be able to get there for their morning dive at 8:30am. Bahama Divers - Diving
There's also another new(er) dive operator now - can't recall the name.

On St. Thomas either Admiralty or Blue Island Divers can pick you up at the Havensight cruise pier. If your ship docks at Crown Marina, you can walk to BID.
