Hey,
Finishing up my rescue class and I'm preparing the E.A.P. Required info is pretty straight forward but I was wondering if there are some "templates"/cool formats out there that are preferred.
Any one have one or know of a place i can go to download. If not, no big deal, can make one up...

There are a number threads that have been active on this same subject over the past 4-6 months, you might find a download (or another student who has done the exercise who will email you their outline) in these threads.

Biggest issues will revolve around communications, locations, and directions.

Regardless of what the emergency is, beyond the level of 'immediate first aid' you'll need to notify other resources. Phone numbers, cell phones, and local medical evac / chamber contacts are right up there. Addresses for dive sites (so people can enter the address into a Nuvi/Garmin/ GPS-type navigational unit) are important to reduce response time. Phone numbers of the victim - family, wife, parents, etc. are also important.

Having a comm plan and a efficient means of notifying people and getting them to the location of the victim are the most important (after initiating local first aid). You might find some other noteworthy items here:

When I took my rescue class I just wrote mine up in an easy to read format. I didn't have any templates. I'm sure each store/agency has some sort of template but I haven't seen them.
 
How'd you get on Diver Jim? Having a similar problem with my plan, what do I leave out is the biggest question!

Never did find one so I just made one up with simple headings and info... seemed to work ok. Good luck!
