Do I need doctor's signature?

Getting ready to start our IDC and in the list of paperwork, etc. to bring the first day our instructor lists a Padi medical form/waiver signed by a doctor. I could have sworn you only needed the form signed by a doctor if you answered "yes" to any of the conditions on the form... Am I confused, or do I need to hope I can get into the doctor's office asap to have him sign a form even though I answer NO to all the questions? :confused:
 
For recreational levels, your own okay is sufficient, but for professional levels, you need a doctor's okay.
 
(sigh)not what I was hoping to hear (since that logic doesn't make sense-not your's, PADI's)...

If it's within a year, it should count, from what I understand.
 
a ray of hope! is this something our IDC instructor can verify through PADI or will I need a hard copy to show- our DM class was at a different location than this class..
 
PADI Instructors require an annual medical? (Not disagreeing, just hadn't heard of it before.)
