Does United require it instead of passing the forms out on the plane? They didn't when I flew to CZM in April.

Personally, for me I found that dealing with the form on line was more of a hassle than filling it out on the plane.
 
So is filling it out by hand on the plane. Either way it is a very minor issue.
The last few times I flew in CZM they had no forms on plane. Only in customs line and a total pain filling in standing up with no support for form.
 
When we went earlier this month we were able to get them on the flight, but they ran out quickly.

@jbjclj, do you download and print the form to give them as you leave the baggage area where you hit the button?
 
There's no way that I'd assume that the form would be available on the plane.
Well, OK, but I have been to Cozumel many times and I do not remember any time when the forms were not passed out on the plane. I'm not saying that it never happens, but it has never happened to me.
