Yes, there are numerous conditions that you must meet in order to receive your $100 or $200 Air Travel credit along with the $140 Hotel Credit & One night Free.
On the Air Credit form, the Terms are stated at the bottom: Terms of offer. This $200 Air credit forms part of the Curaçao Now promotion. Offer valid for residents of the United States and Canada. Your travel must be booked before July 15, 2009 for travel between May 01 - July 01 or Aug. 15 - Dec. 15, 2009. Maximum of 2 Air Credits per room.
Minimum stay of 5 nights required at one of the Participating Hotels mentioned in the program.
The participating Hotels are listed at: <http://www.curacao.com/now/hotels.html>
This is why you must enter your Hotel Name and Confirmation number on the credit form. All the conditions are disclosed ... you just have to navigate to the different pages of the program site.
And with your passport, they are only looking for confirmation of your identity that you are either a US or Canadian citizen.
On the bottom of your passport picture page, there are 2 lines of information with your Country and Name, followed by many characters and numbers. Example: P<USAYOURLASTNAME<<YOURFIRSTNAME<<. After the chevrons, is your passport number, dob, doi. When you make your copy, just take a black marker and cover up everything after your country and name. Also cover up your Passport Number in the upper right hand corner. This procedure should allow you to feel a bit more comfortable with transmitting this information.
I know this has been frustrating for some. The fine print always gets ya ... Yes, there are many folks that have benefited from this program and have received their rebate. Sometimes it takes 6 weeks to get your check. Persistence does pay off with this.

Ours also took about 7 weeks to arrive. Just followed the directions on the website.
When are you going to be in Curacao and where are you staying?
We will be there Dec 5-12 - staying at Lodge Kura Hulanda in Westpunt. Both my wife and I will be diving.

I filled out the form under Curacao Now, "$200 Air Credit", with booking date (11Jun09) travel dates 6-13Dec; Breezes confirmation #, airline reservation locator. etc--nothing about passports. We will be diving w/Ocean Encounters. We have calls in to Mr. Rojer as well as emails.

I just checked the dates and I emailed mine on June 17 and got the check last week.
Did you include a copy of both the airline booking and hotel confirmation letter - or just the booking numbers?
I sent copies (pdf) of both attached to my form. I got confirmation within 24 hours that they were received and everything was complete.

Curaçao with except of the Sunquest / Thomas Cook charter service from Canada.

the required documentation. The CTB is not responsible for late, lost, illegible, mutilated,
incomplete, misdirected or postage-due mail. CTB reserves the right to appeal for full
reimbursement in event of cancelation or postponement, regardless of circumstances.

I finally got someone to talk to at Curacao office, they said that I should contact Caradonna who did the booking--I did & they said that the credit had just been taken off the total package price, so I guess we're taken care of. Muzicgirl did you also book thru a company or just hotel & air separately--could be the same situation. Now we can relax---- till the final payment.
