I have just spoken on the phone to a guy i did my open water certification with, i asked him if he'd done any diving since the course but he says he is too afraid to as
he found blood inder his fingernails (in the corners)after the pool sessions. he says he just clipped his nails and scrubbed the blood away. he had no bleeding after the ocean dives and he didnt tell the instructor. he has no history of hypertension. he refuses to see a doctor but also refuses to dive again in case there is something wrong.

Why does he not want to visit a physician? It would certainly help allay his fears, if he indeed has fears about being medically fit to dive.

Ignoring the blood under the fingernails bit, I wonder if there are other apprehansions he has that are not yet spoken....
 
Blood underneath the fingernails...that's a different one.

But could be explained by tugging on a wetsuit (did he try one on before the class) - when you accidentally pull your fingernails back..

was it one fingernail, or them all? Certainly there will be a rational explanation for it - but if he's too scared to see a Dr. - IMO, this may be just a sign that he didn't really like it and its a good excuse to "politely decline".

Usually those who want to do it again will be proactive about solving any issues - stuff like this gives them an easy way out while saving "face"...
