Every accident is a chain of events and not a single cause. There are millions of lionfish stings worldwide every year (I've had at least a hundred). A fatality is extremely rare and usually with contributing factors. I don't think all the details have been published on this accident.I had no idea they could be lethal.

Seems like it. Also important to note that lionfish only recently have invades northern Brasil and probably not everyone is aware of the danger they pose.Most likely allergic to venom and died from anaphylaxis.
