Azimuth Death in Asia

Does anybody have any info on the Azimuth death in Asia. I believe it was either Malaysia or Indonesia where the incident occured. This is the second rebreather death I have heard of in the last month.

Never mind, that was a Draeger Ray. So that would be the third RB death in a month. They say it was a heart attack.
