I'm considering buying one of these for my new AL Legend LX. Opinions on whether or not it's worth the $89. Or is it more or less just another failure point and waste of money.

Not to me.

If you're having problems with jaw fatigue then you might try replacing your regulator hose with one that is more flexible and/or longer. Should save you anywhere from $30-50.
 
....well, I've installed the Atomic swivel hoses on 8 of my reg sets, so I guess they must be worth it....

I have the Atomic Swivel hose on all three regulators that my family owns (two Atomic & one ScubaPro).

My wife has TMJ and has no problem with the Atomic swivel hose.

Both my wife and I dive Atomic regs with first and second stage swivels She used to complain of jaw fatigue with her old Sherwood, and swears by the Atomic swivel. It was never an issue for me, but she's happy so I'd say it was well worth it.

I've never heard of one failing, btw.
 
I have TMJ by the way. I have to open my jaw almost wide enough to eject the regulator to equalize by shifting my jaw side to side - otherwise the jaw completely locks - or clicks pretty bad. I don't always need to do that to equalize - but it does make it easier. My TMJ is pretty bad - but somewhat better if I wear the sleeping device every night religiously - bite guard.
 
I have the Atomic swivel on my main second stage and a miflex hose on my backup second stage. I can't really tell much of a difference breathing out of either. Neither seems to pull. I would go with whichever is cheaper to achieve the same result. That said, the miflex does fold up more easily. So if price is similar, go with the miflex.
