Unless there is actual blood in the water Padi will do nothing. You're wasting your time just name the shop here and find someone else to patronize
 
As I read the complaints and take them as liberal as possible, I can only see one real issue, #1. #4 does not cause any harm. #2 can be misinterpretation of a friendly staff trying to include customers, maybe not good professionalism, but not a violation. #3 while rude, is again not a violation of any standard.

Now, we can address #1 as the only real potential problem. Without a second opinion, this is only as single interpretation of what actually occurred. PADI and every other agency will see it this way as well. Included with the other three complaints makes this seem less actionable. By itself it may have occurred as stated, but maybe a bad day for the captain. A single incident does not warrant punishment.

So, there is not much you can do other than not patronize that establishment. Even if you listed the operator here, I would expect several folks who have had good experiences to reply and negate your single bad experience. The best you can do is drop it, forget it, and move on to another operator that will satisfy your desires. Preserve your insanity and realize that not all operators operate at a level you might expect. Keep diving!
 
I see no actionable QA items for PADI to respond to. I do see that the dive shop you were going with has issues and I encourage you to reach out to the management and staff of that dive shop and register your complaint.

You can also use TripAdvisor and also name the dive shop on social media to put pressure on them to take their jobs seriously.

Is there a reason you did not name the shop/boat in your posting here?
