My second to last dive when I was working in Bonaire a few years ago. Got lucky on the east coast.

I'd say Bali would be a good starting point. Culture, shopping, luxury resorts, great food, good diving. Well developed tourist infrastructure so not too adventurous to get around.
Maybe I'm not understanding the question correctly, but every diver on the Kittiwake should have a token. It is generally included in your dive fee and should handed out by the operator to be attached to your BCD.

A month of CCR diving in PNG sounds like a bucket list dive trip! I sailed for a month doing rec diving of a sailing ship... best diving I've ever done.
