Even breathing off the reg while inflating a BCD or drysuit at the same time can cause a freeflow. Of course a stressed diver sharing air is definitely even harder on it. My understanding is that as the air comes out of the tank and goes through the reg there is a significant temperature drop...
I agree with Ayisha ... the Forest City is extremely cold and last September I had a temperature of 39 degrees at depth, so a TUSA that is certified to 38 degrees is just a bit too close for me. I do dive a Scubapro MK25/650 and have experienced some issues with 2nd stage freeflows but thus far...
In my books - that is your problem. I don't exactly look at TUSA regs as being great cold water regs. I'm sure others will disagree but in cold water I stay away from their regs (although I do use their gear in other environments).

4 mm is actually more of a compressed neoprene then a true crushed neoprene. The suit I wear is a 2 mm crushed neoprene. Benefit of the 2 mm is less lead requirements then a 4 mm but still more thermal value then a trilam.
