I posted the incident report that was sent to PADI. Without any media coverage, there is limited information available. I attempted to make what little information I had access to available for discussion.
I have access to the PADI incident report. Would providing that be acceptable? It would show that what was in the official report was the same as what was reported on Trip Advisor.

I notice that Nick states that he started CPR, but from my understanding, it was another person that started CPR after the passenger turned blue. I wonder if Nick is conflating administering O2 with starting CPR.
I agree about not mixing CCR. I know the CCR divers that were on the boat and they have told the dive shop owner to leave them if there is an emergency while they are down. It doesn't sound like this was communicated to the staff.

I see the main issues as the dive op left customers to manage the situation, had faulty oxygen on board, and did not have a procedure in place for what to do if a recreational diver had an emergency while the rebreather are down.
