I once dove off a NC vessel when there was an incident. The O2 bottle was small and almost empty. Fortunately, no one died.
But the next several times I dove off the NC coast, I brought my own O2 kit.
It happened on Saturday. I was not there but was there next day. Word is that it was a tech diver. Victim was male.

As a practicing attorney, I deal with the issues of waivers from time to time.
There are many factors that can be involved, including the jurisdiction in question. In the US, waivers rarely, if ever, prevent a case from going to trial if there is an accusation of gross negligence.
Please know that in the event of a lawsuit, there would likely be more than one defendant. PADI would likely be a co-defendant as might the company that owns the quarry. So, there would be multiple deep pockets here.
