Without a doubt, you want the BC first. The others have all given good reasons, and I'll add that you want to be comfortable. Getting a rental BC that fits and is comfortable happens about half the time.
You may want to get him to fill a sink with water, put the snorkel in his mouth and then put his face in the water and breathe through the snorkel for several minutes several times. It helped me when I first started diving.
Those knowledge reviews must be reviewed, corrected, gone over with the student and a copy (or original) must be placed in the student's folder which is kept by the instructor or LDS. If your instructor did not do these things he is in violation of the standards and should be reminded of such.

I will do some more investigating, but I do not believe PADI will require DAN O2 but they are going to require an intro to O2 provider. PADI offers and O2 course as a distinctive specialty so it would be counter productive of them to require DAN's course.
