I use a bit of both. Some weights integrated in the BCD and some on my belt. Not enough weight on my belt to be negatively bouyant without the BCD. Also use a steel tank which reduces the amount of lead I have to carry.

Poster was rather inexperienced with only 20 dives swimming in current and warned DM low on air at 20 minutes. My experience with DM guided dives is that DM kept track of divers air supply and also asked if divers are OK at regular intervals.

Similar thing happened to my wife. She was about ten feet deep and wanted to get deeper. She pressed the wrong button and went up instead. There is the possibility that the BCD was actually ok but that operator error could have been involved.

Had a similar experience trying to get a dive knife out of knife holder. The design allowed sand to enter the holder where the release mechanism had to bend in order to work. I modified the holder with some silicon rubber to prevent sand ingress. Switched to a different type dive knife later.
