The only time it happened to me was a slow leak. I just kept dumping the excess air until the dive was over and then replaced the inflator with a spare. It did take a little while to figure out what was going on, though.

Inflator or zipper. I'd suspect the zip not being fully seated at the bottom or your inflator being loose. Take the inflator off and ensure nothing is between it and sealing surface. Next time you get in the suit pull the zip down firmly once you're at the bottom to ensure it is seated.

Take the inflator valve apart. Note if it's screwed together tightly when you unscrew it. Make sure any gasket that is supposed to be there is in fact there. Put it back together and make sure it's screwed together tightly. Try again.

This is the way to find the leak. Flip the suit inside out and the same with the inflator. Use a ball or bucket to seal the neck and cups for the wrist. pump it up nice and tight hose down with soapy water. how you repair it from there is up to you.
