Is this just a label, or truly for oxygen use ?

The answer is, it depends. Is it a new reg? If so, it should have been O2 cleaned at the factory. If a used reg, then who knows? If it's new and has never been put on a cylinder that used air that was not compatible for O2 use, in theory it should be ok for use with 100% O2.
It's a deco reg I'm guessing but not sure why it has a yoke assembly instead of DIN.
 
Someone ordered that weird being yoke and not DIN but I don't use HP O2 bottles........ We are supposed to be planning for lost gas so...... I'd use it.
