As in a buddy would have said "have you lost your f$%^ing mind?" after seeing what gasses and how little she brought to the planned dive?
Is that confirmed? Sometimes you can have something on your mind or a distraction that occurred much earlier and coupled with a belief that this is just another dive of many, certain details can be missed, or if you were in a hurry when some unexpected delay put you behind your schedule.

Carbon Monoxide? Who did her fills, Buddy Dive?
 
And why didn't it affect her earlier in the dive?
 
Wow. Another twist of the story. CO poisoning. I haven’t heard that one for awhile.
 
The line that talks about carbon monoxide comes directly after the statement from a quote saying "the combination of air and depth in regard to safety were not rational". Which leads me to think the representative from Buddy Dive may have speculated about carbon dioxide, as a product of too much work of breathing, and somewhere in the publishing process it became carbon monoxide.
 
The coroner's report should have the full story, if it is made public. Older divers have succumbed to heart failure when an unsuspected underlying condition has been overtaxed due to a sudden exertion. There are a whole bunch of things that can come out of the blue to knock you off and although rare some people can take a turn and inexplicably at the time just drop dead.
