Failure of back-plate single tank adapter (Bolts)

It's more likely the nuts loosened and fell off than another failure like the bolt breaking off. Look into lock washer, nylock nuts, or checking every dive.

I don't use an sta so cambands go through plate so don't have same issue.
 
Do you have a pad covering your backplate? Maybe consider ditching the pad, so you can simple see if nuts are loose or missing?

And no, the bolts are extremely unlikely to break. Songle AL80 is so light weight compared to what the bolts can hold. Just imagine people put on double LP118 with the same diameter bolts.
